Life Care Planning

What is a Life Care Plan?
The Life Care Plan is a comprehensive program which we develop specifically for you to assess your current situation, and provide you with an action plan addressing your needs, be it physical, emotional, and legal.   This will be a fluid plan, changing as your circumstances change. 

How Does This Work?
Our Life Care team will have an introductory meeting with you, your family and/or trusted advisors to discuss your goals and the Life Care Planning process.  Following that our Life Care Advocate will meet with you in your home to conduct a thorough assessment of your current situation, focusing on your medical, emotional, and safety needs. This comprehensive assessment will provide our Life Care Team with the ability to formulate an individualized Life Care Plan that will specifically target concerns, identify additional needs, recommend and manage resources from our vast professional network.

The Life Care Plan is a fluid plan that will continually evolve.  As such, our team will work with you and your family to provide ongoing monitoring and assessment, recommendations and oversight of qualified services for your situation, conducting regular reviews.  Our pro-active team approach is here to plan ahead, support you and be there in any crisis.   

Why Consider a Life Care Plan?

  • Are there disagreements within the family on care needs?
  • Have there been recent health or behavioral changes?
  • For couples, is one the caregiver and the other having health care issues?  If so is the caregiver given support too?
  • Are there multiple physicians treating multiple conditions? Is there coordination between them?
  • Is your home set up safely or do adaptations need to be made?
  • Have there been recent accidents or falls?
  • Is a move needed to a safer environment?
  • Do you have the correct insurance coverage?  Should you consider long-term care insurance?
  • How can you plan to preserve your assets as you age or need assistance?
  • Do you have the requisite legal documents in place?
